DHC – Health Care Administration

The Minnesota Department of Human Services (DHS) Health Care Administration is responsible for overseeing the development, operational components and federal and state compliance of Minnesota’s public health care programs, including:

• Medical Assistance (MA, or Minnesota’s Medicaid program), which provides coverage for an average of more than 660,000 low-income people each month, including children and families, people 65 or older, people who have disabilities and adults without children.
• MinnesotaCare, which is designed for Minnesotans who do not have access to affordable health care coverage. MinnesotaCare serves an average of 148,000 people each month, and can be critical to helping people leave welfare and go to work without losing health care coverage.
• Healthy Minnesota Contribution Program, which began July 1, 2012 and provides a subsidy for adults without children at the upper end of the MinnesotaCare income range to purchase private health care coverage in the individual insurance market.

Through these programs, the state pays all or part of enrollees’ medical bills. In FY 2011, about 74 percent of DHS’ all-funds (state and federal funds) budget was devoted to these programs. These programs provide a safety net for low-income families, the elderly, disabled and very low-income adults without children. When faced with no coverage, individuals often seek preventative care services in an emergency room setting or choose not to seek preventative care. Uninsured Minnesotans also receive care in an uncoordinated and uncompensated manner in disparate provider settings (as opposed to a primary care setting), and/or delay seeking health care for emerging conditions. All of these choices result in reduced health outcomes for Minnesotans and/or increased cost for taxpayers.

The goals of the DHS Health Care Administration (HCA) are to:
1. Reduce the number of uninsured Minnesotans, by helping eligible people get health care coverage and measuring the quality of this coverage to improve it;
2. Reform the payment and service delivery models for Minnesota’s public health care programs, by designing our rates and systems to be transparent and of maximized value for the taxpayer dollar;
3. Utilize research, data and analysis to develop HCA initiatives, support DHS health care programs and evaluate results;
4. Employ technology solutions to reduce costs and improve services for applicants, members and providers; and
5. Encourage stakeholder communication across HCA, to promote understanding and support of Minnesota’s public health care programs.

HCA is actively engaged in implementing federal and state health care reform initiatives, to maximize available opportunities and promote health reform.

Strategies
HCA’s operational components include eligibility policy, provider and member enrollment/relations, federal relations (with the Centers for Medicare & Medicaid Services, or CMS, which administers Medicaid federally), managed care enrollment and contracting, project management, purchasing and service delivery, claims processing, benefit recovery, system development and maintenance, performance measurement, and numerous other functions of a public health plan. (HCA shares some MA and MinnesotaCare coverage policy and rates development functions with the DHS Continuing Care and Chemical and Mental Health Services administrations for those services.) HCA also develops improvements to the way we administer and deliver these programs.

DHS and HCA work with many stakeholders to help us determine how we can improve our health care programs in the following ways:

Improve health outcomes of public health care recipients while lowering costs for taxpayers
• MA and MinnesotaCare provide coverage of care coordination services for people with chronic illnesses through physician clinics that are certified Health Care Homes.
• DHS, per legislative direction in 2011, is seeking federal approval to reform MA to achieve better outcomes for people with disabilities, seniors and other enrollees through the Reform 2020 initiative. More information is available online at http://www.dhs.state.mn.us/main/dhs16_166654
• DHS is submitting a grant application for a CMS Innovation Challenge for the “Strong Start for Mothers and Babies” initiative, to decrease disparities in birth outcomes for infants from communities of color.
• DHS is working with many partners to launch the Money Follows the Person Rebalancing Demonstration Program, aimed at increasing the number of Minnesotans with disabilities served in their homes and communities rather than in institutions. (See http://www.dhs.state.mn.us/dhs16_162194
• DHS is partnering with the Minnesota Department of Health to help MA and MinnesotaCare enrollees prevent diabetes and other chronic disease by participating in the YMCA’s Diabetes Prevention Program and receiving other incentives for healthy behavior. Funded through a five-year grant, this program will help clients improve their health while lowering state health care spending for chronic conditions.

Integrate primary care, behavioral health and long-term care
Minnesota will see large growth in cost and utilization of long-term care services as Minnesota’s population ages. We can save money, reduce duplication of services and improve the health of our recipients if we better integrate our health care services. Some examples include:
• As part of Reform 2020, DHS is building on current state initiatives to improve performance of primary care and care coordination models for dual Medicare-Medicaid eligibles. Part of this focus is on primary care payment reform, and the consistent measurement and improvement in clinical outcomes. • DHS is developing a grant project to integrate primary care and behavioral health care.
• DHS is partnering with Hennepin County on an innovative pilot project to test new strategies to better deliver human services to high-needs populations. This new demonstration, Hennepin Health, will provide a system that integrates medical, behavioral health and county human services for up to 10,000 low-income Minnesotans in a patient-centered model of care, improving patient outcomes while reducing overall costs.

Institute payment reform in health care purchasing
HCA is negotiating with potential Health Care Delivery System (HCDS) sites to begin full operation by January 2013. HCDS demonstration projects will contract directly with providers in a new way that allows them to share in savings for improving quality of care and patient experience and reducing the total cost of care for Medicaid enrollees. In this new model, MA enrollees will receive more coordinated care to improve their overall health, and health care providers will be paid based on the quality of care they provide to their patients and their ability to reduce the cost of care.

HCA’s work also supports the following strategies in the DHS Framework for the Future: 2012 (https://edocs.dhs.state.mn.us/lfserver/Public/DHS-6464-ENG):
• Increase access to affordable health care
• Integrate primary care, behavioral health and long-term care
• Institute payment reform in health care purchasing
• Use technology to increase our outreach through online applications, a new website and e-licensing initiatives
• Narrow the health care quality gap between clients in publicly-funded health care and private enrollees
• Increase enrollment outreach to communities of color

Results
Minnesota is consistently a national leader in promoting and implementing policy and payment initiatives that improve access, quality, and cost-effectiveness of services provided through publicly funded health care programs. We monitor performance measures that help us get at key actions and strategies. If DHS can quickly reimburse providers who serve our recipients, these providers may be more apt to serve recipients of Minnesota’s public health care programs. Timely disposition of applications greatly improves access to health care coverage for eligible customers. Treating people in emergency rooms is more expensive than keeping them healthy to begin with, so it makes sense to get people the primary care they need. This last performance measure needs improvement, since providing people with insurance offers access to the kind of preventive care that keeps costs down and helps people live better lives.

The City of Brooklyn Park, Minnesota’s sixth largest municipality is seeking applicants for seasonal intern positions within the Community Development Department. EXAMPLES OF ESSENTIAL FUNCTIONS: Conduct property maintenance inspections and enforce the city public health, zoning, sign, nuisance, and property maintenance codes Conduct and/or assist with inspections of licensed food establishments, public swimming pools, spas, licensed lodging facilities, and itinerant food events; obtain food, water, and environmental samples. All public health related activities are conducted under the supervision of a registered sanitarian (these duties require studies in an Environmental Health related field) Issue correction orders to property owners and take legal actions including issuing administrative fines and ordering abatements; answer questions from the public; perform special projects as directed Conduct various administrative functions including answering phones, data entry, imaging documents etc.

The NAZ Promise Neighborhood is a collaboration of over 50 organizations and schools who work with children and families in a geographic area, or “Zone” of North Minneapolis to build a culture of achievement so that all youth graduate college ready. We will do this by partnering with schools, organizations, and families to significantly improve education and guarantee opportunities to support for children from pre-birth to college.

Position Overview:

The Program and Clinical Manager leads a team of NAZ Connectors who are the primary contact for NAZ families. The Program Manager is responsible for ensuring the success of the team by providing training, coaching, and problem-solving to Connectors. The position is also responsible for creating collaboration among the Engagement Teams, service Navigators and Academic Navigators.

As a Clinical Manager, this position informs, supports, and reinforces behavioral health strategies within all program teams. This position will participate in leadership team meetings to inform the full engagement process, provide clinical supervision to the Program Managers across all teams, and provide clinical consultation related to NAZ families in all program teams.

Primary Responsibilities:

• Engagement Team Management: Manage one team of approximately 8 NAZ Connectors within the Engagement Team to support outcomes for NAZ families and students. Participate in strategic and operational planning meetings focused on effectiveness of the Engagement Team. Promote innovation in all programming. Participate in the development of processes and protocols that promote effective engagement in NAZ of families and the community.

• Program Development: The Program and Clinical Manager will work with NAZ leadership to support program development and the implementation culture across all program teams.

• Clinical Supervision and Consultation: This position will provide clinical supervision to other Program Managers, and case consultation within case planning meetings across all teams.

• Staff Supervision: Develop and oversee a team of NAZ Connectors and Lead NAZ Connectors with a focus on achieving results. Supervision responsibilities include managing day-to-day workload; monitoring and supporting the effectiveness of their work through ongoing evaluation, real-time feedback and progress tracking tied to family outcomes. Supervise Lead NAZ Connectors to also serve as a mentor and coach to NAZ Connectors within team.

• Partner Collaboration: Promote effective collaboration between NAZ Connectors and program partners by developing of shared goals within family achievement plans. Promote effective engagement of partners in these achievement plans.

• Results-Orientation: Foster a culture focused on achievement within the Engagement Team. Ensure that NAZ Connect is fully utilized to promote and monitor achievement. Use data offamilies and children in NAZ to drive workload. Document and report on the progress of staff, families and partners.

• Other duties as assigned.

Supervision Responsibility:

• Clinical supervision of Program Managers (under the leadership of full supervision by Engagement Director)

• General supervision of NAZ Connectors and Lead NAZ Connectors

Job Qualifications:

• Independently licensed clinician (LICSW, LP, LMFT) with a minimum of 10 years experience providing therapeutic services to children and families and a minimum of 5 years of experience supervising clinical staff.

• Experience providing supervision and program management to diverse staff required.

• Experience working with diverse populations, providing home based services, training in evidence based practices also required.

Required Knowledge, Skills and Abilities include:

Position Overview:

The Executive Assistant provides administrative support to the president and board members, and responds to communications on behalf of the president and board. S/he schedules meetings or directs the individual or group to someone within NAZ who may better assist them. This position records and writes the minutes of board meetings and understands the importance and need for tact and confidentiality.

Specific Responsibilities:

• Serve as first point of contact for outside callers; determine appropriate action. • Make travel arrangements including registration for conferences, hotel accommodations, air travel, provide supporting materials for President’s travel. • Draft memos, letters, or other writing as assigned. • Schedule meetings, keep calendar up to date and accurate; provide daily schedule with backup information for President. • Coordinate meetings, including staff and board leadership retreats, conference calls, and sessions as requested by President. • Sort mail; route as necessary; respond or follow up as necessary. • Maintain Contacts list, files, correspondence. • Prepare expense report for President, visa bill and other invoices. • Manage long term and short term projects.

Support of the Board of Directors

• Make travel arrangements including registration for conferences, hotel accommodations, air travel, provide supporting materials as available. • Maintain Board Committee schedule; send reminders • Serve as first point of contact for outside callers for the Board Chair; determine action. • Assist board members with basic tasks such as notarizing, copying, sending materials, researching a question. • Assists board members with special projects. • Act as liaison between board and staff. • Provide board committees with administrative support.

Board Meeting Responsibilities

• Record and write board meeting minutes. • Prepare draft agenda, attendance sheet, feedback forms, others as assigned.The qualified candidate will have an AA degree and a minimum of five years related full time professional work experience in an executive/board support position in an organization of similar size, or a combination of equivalent experience and training.

Experience working with a board of directors is strongly desired.

The NAZ Promise Neighborhood is a collaboration of over 50 organizations and schools who work with children and families in a geographic area, or “Zone” of North Minneapolis to build a culture of achievement so that all youth graduate college ready. We will do this by partnering with schools, organizations, and families to significantly improve education and guarantee opportunities to support for children from pre-birth to college.

Position Overview: The Education Director provides leadership, supervision and training to the Academic Navigators as they provide “high touch” support to students by tracking and monitoring their performance, establishing individualized academic achievement plans that support college-readiness at every grade-level, and partnering with students and their families to follow-through on those plans. The Director coordinates and supports education and training for school leadership and staff, and serves as a connector and partner to the other NAZ service providers along the education pipeline, from early childhood, to K-12 school-based, to out of school time, to college. This will ensure the effective coordination of high quality services and supports along the continuum of a child’s life.

The overall goal of the Education Director is to prepare all children in NAZ for college by providing expertise and knowledge in improving academic achievement for all students in NAZ with a special focus on those most at-risk of academic failure, and to help design and implement systems within NAZ that create a culture of academic achievement within families, schools, students and the broader community.

Primary Responsibilities: Education Team Leadership, Management and Supervision:

§Provide strategic and operational leadership for the education-focused components of NAZ to ensure that all NAZ students increase their academic performance and are on track for college.

§Promote results-orientation and innovation in all programming.

§Develop processes and protocols to facilitate “high touch” support to students and families in NAZ.

§Work in coordination with school staff, Academic Navigators, academic out of school time partners and parents to monitor students’ academic progress and address behavioral issues.

§Meet weekly or more frequently as needed with cohort teams to review family data, track progress and identify actions to support family goals.

§Work with Engagement and Project Services Director to develop program implementation pathways across teams to drive family achievement outcomes.

Partner Collaboration:

§Develop trusting relationships with school staff, district administration and parents within NAZ.

§Collaborate with schools and college-readiness partners to develop a NAZ-specific curriculum for use in schools, out of school time, and parent education venues.

§Coordinate and support training for school staff in accordance with the school-based strategy outlined in NAZ Plan.

§Partner with district administrators and school leadership to facilitate agreed upon education transformation strategy through the zone schools.

§Participate in meetings with other Action Team leads along the Education pipeline to ensure coordination of high impact strategies for students in the zone.

§Develop and oversee a team of Academic Navigators that are focused on achieving results. Supervision responsibilities include managing day-to-day workload; monitoring and supporting the effectiveness of their work through ongoing evaluation, real-time feedback and progress tracking tied to student outcomes.

Job Qualifications:

Minimum Education and Experience

MA degree in education, public administration or related field; and 5 years experience in leadership role within education sector of similar size, or a combination of equivalent experience and training

Position Overview:

The Accounting Assistant maintains and keep records of all organizational financial systems, reconciles and balances accounts, prepares budgets and reports on a regular basis, interfaces with vendors and program partners, assists with the preparation for regular auditing and monitoring, and provides cheerful professional customer service to the internal and external users of NAZ’s financial systems.

Primary Responsibilities:

• Customer service: provide cheerful and professional service to the internal and external users of NAZ’s financial systems.

• Accounts payables: verify all invoices for payments, process payment transactions ensuring timeliness and accuracy of information and appropriate backup documentation. Maintain communications with vendors.

• Accounts Receivables: prepare invoices, send statements, monitor receipts and the ageing schedule, ensuring timeliness and accuracy of information and appropriate backup documentation.

• Payroll and timesheets: prepare, process, transmit, and post bi-monthly payroll. Post timesheet detail data to the general ledger bi-monthly.

• General Ledger: maintain general ledger accounts; prepare and post journal entries, including accruals and pre-paid expenses, timesheet data, in-kind expenses and revenue; ensuring timeliness and accuracy of information and appropriate backup documentation.

• Deposits: Post bank deposit receipts, ensuring timeliness and accuracy of information and appropriate backup documentation.

• Bank reconciliations: prepare bank reconciliations on a monthly basis, ensuring timeliness and accuracy of information and appropriate backup documentation.

• Regular reports – for staff, board and committees: perform month-end and year-end close out procedures, prepare financial statements.

• Reports – for funders: when requested prepare program-specific or organizational financial reports.

• Budgets – organizational and program-specific: as requested prepare drafts of organizational or program budgets.

• Debit and/or credit cards: Post activity and ensure proper documentary support. • Cash Flow: Prepare and update monthly cash flow analyses.• Filing: organize all receipts, invoices, and other supporting documentation and file weekly according to established procedures and the principles of sound, responsible financial reporting.

• Assist with the preparation for annual audits and federal monitoring. • Other duties and special projects as assigned.

Job Qualifications:

• Experience working with accounts payable, accounts receivable, preparing and maintaining payroll using a payroll service, reconciliations, journal entries, reporting, audit preparation.

• Ability to plan and organize work, set priorities and meet deadlines.

• Ability to make sound, independent judgments within established policies and procedures.

• Respect for mission of the Northside Achievement Zone and its expectation that children growing up in North Minneapolis can succeed in academics and college.

Minimum Education and Experience

• The qualified candidate will have Associate’s degree in accounting or bookkeeping and three years experience in similarly comprehensive accounting position, or a combination of equivalent experience and training.

• Understanding of and experience in complying with Generally Accepted Accounting Principles (GAAP). • Experience with QuickBooks accounting software. • Knowledge of other accounting systems and/or ability to learn. • Use of the Microsoft Office suite, with special expertise with Excel. • Experience with on-line payroll reporting services. • Appetite for customer service. • Nonprofit accounting experience strongly preferred. • Ability to communicate effectively and respectfully with a variety of people and groups, particularly northside neighborhood leaders, organizations, schools, churches, and families. 2012 First Step Summer Institute

Greetings!

It’s that time of year again! I’m looking to recruit a stellar class for the 2012 First Step Summer Institute at St. Catherine University (St. Paul, MN), and I would appreciate your help getting the word out to students. I imagine that you know someone–your daughter, your sister, your cousin, your niece, your classmate, your student, and/or your neighbor–who would really benefit from First Step. The First Step Summer Institute is a free, unique college-immersion experience that prepares 50 young women of color for higher education. In each one-week residential program, 25 students gain a deeper sense of commitment to pursue higher education, a strong network of diverse, college-bound friends and mentors, a sense of support and encouragement from St. Kate’s staff, faculty, and students, and more perspective on college living and college expectations. Plus, students have a great time exploring campus and the surrounding Highland Park area.

If accepted to First Step, students have the option of attending one of two sessions:

* Session I: June 18-22, 2012
* Session II: June 25-June 29, 2012

By participating in First Step, students will get:

* Instruction from college staff and faculty about career exploration, ACT test prep, financial aid, essay writing, scholarships, study skills, college culture, women’s empowerment, and college money management
* Advice from students of color attending St. Kates and professional women of color who are leaders in the Twin Cities
* Fun, interactive, outdoor training on leadership and teamwork skills

All First Step graduates are eligible to receive a scholarship of up to $2,000 per year for four years (totaling up to $8,000), if they enroll at St. Catherine University.

Students must meet the following eligibility requirements in order to be considered for admission to First Step:

* Be currently enrolled as a sophomore or a junior in high school
* Identify as a woman of color
* Have at least a 2.5 G.P.A.
* Submit two completed letters of recommendation (see attached document)
* Submit a completed application (see attached document)
* Submit an official transcript

The APPLICATION DEADLINE for First Step is Wednesday, May 2, 2012. Admission decisions will be mailed to students and families by the last week in May. All application materials should be mailed to me at the following address:

The HR Department is comprised of over 70 professionals providing a range of comprehensive services including Staffing, Benefits, HealthWorks, Classification, Compensation, Learning & Development, and Organizational Development for more than 7,500 employees.

The primary duties and responsibilities of this position include:

  • Assist the Organizational Change & Effectiveness Manager in the development,implementation and evaluation of change management strategies and plans for large, cross departmental change initiatives.
  • Design and deliver change management training and development programs.
  • Coordinate with department leaders, stakeholders and project teams to foster buy-in, support organizational readiness and monitor progress.
  • Partner with experts in other areas to provide assistance with department-specific change management efforts.
  • Identify strategies, methods, tools and measures for effective organizational change that can be applied by departments in change management efforts.
  • Identify communication and training needs and develop methods to engage target audiences throughout the change process.
  • Work with other Human Resources staff to address workforce issues and provide coaching support for department leaders related to leading change.

Best Qualified Candidates will have:
One of the following:

  • A bachelor’s degree or higher in organizational development, human resource
    development or other related behavioral sciences field PLUS three or more
    years of organizational development or change management experience.
  • An equivalent combination of education and related experience to equal at
    least seven years. Experience may be substituted for education on a
    year-for-year basis.

Three or more years of experience in leading and consulting on organizational change initiatives.
Experience applying change management theories, concepts, and approaches in business situations.
Experience designing/delivering training and facilitating group discussions
regarding department wide change initiatives.
Knowledge of leading-edge organizational change and development concepts, methods,
and tools.
Knowledge of adult learning and development concepts, methods, and tools.
Knowledge of organizational behavior and team/group processes.
Ability to interact and communicate with, and effectively coach multiple business
partners including senior level executives and managers.
